A principle that is true for many risk-lowering strategies, is that the relative risk reduction achieved with LDL-c lowering is similar across various subgroups with different risk levels. If the absolute risk is higher, however, the absolute risk reduction of lowering LDL-c is greater. Thus, the most aggressive treatment with PCSK9 inhibitors, which is also the most costly approach, should be targeted in the highest risk groups.
